| Skilled Nursing |
| Southwest LTC strives to offer the very best care at each of our facilities. We feature skilled nursing and rehabilitative care, as well as
traditional long-term care services. |

| Skilled nursing is for those residents who need 24-hour nursing care and those who are in short-term rehabilitation following a hospital stay.
Rehabilitation is also provided as needed on both a long and short-term basis. We offer a wide range of services and amenities, from spiritual care to daily
activities and pharmacy services. |

| Each skilled nursing resident will receive an individualized treatment plan designed for optimum recovery. Goals are set for each resident
by an interdisciplinary team comprised of the resident and/or family members, the participant's physician and our health-care professionals. Weekly team
meetings are held to review the progress of the resident and re-evaluate the treatment plan. |

| Rehabilitation Centers |
| Our facilities keep a focus on rehabilitation therapy. Each facility is staffed with speech therapists, physical therapists and occupational
therapists that routinely screen our patients to see of they can be assisted by therapy. With a focus on geriatric patients, our therapy departments have
expertise assisting patients who have had strokes or fractures recover and return to their homes. For our long-term patients the rehabilitation team is able
to regularly assess them to help maintain function and ability. Through day-to-day contact our staff get to know patients? abilities and habits, enabling
them to make rapid assessments and timely interventions. |

| Once the goals have been achieved or the team determines the resident is no longer progressing, the patient will be discharged home or to an environment that will best meet their needs. |

| Pharmacy |
| Our facilities offer pharmacy delivery 24 hours a day, seven days a week, with an on-call pharmacist available at all times. Through our pharmacy
delivery service, Medicare and Medicaid are billed directly, making receipt of much-needed medication more convenient for our patients. |

| Spiritual Care |
| As we always emphasize, it is imperative that we take care of our residents' physical and emotional needs, but caring for their spiritual side is equally important. |

| Our program offers a non-denominational, all-inclusive approach to spiritual care. Residents benefit from a wide range of religious services
with complete freedom of religious worship. The program offers a comprehensive spiritual plan that provides a range of opportunities for our residents to
participate in structured and non-structured worship and faith-based counseling activities. |
